Skilled Nursing at Tiburon Hills Care Center
At Tiburon Hills Care Center, our Onsite Medical Director coordinates our medical services and our Director of Nursing oversees all aspects of healthcare within our short and long-term care facility to ensure we offer:

Diabetes Management
Our experienced nurses provide help with all aspects of managing diabetes including blood glucose monitoring, insulin administration, and tracking, making an exercise plan, menu planning, education, and more.
Complex Wound Care
Our wound care program utilizes professional, clinical, and technical resources designed for the treatment of a variety of wound conditions including ostomies (colostomy/urostomy), wound separation, diabetic arterial/venous ulcers, pressure ulcers, and pre- and post-flap surgery.
Respite Care
Through our respite program, our facility provides security and supervision in a caring and compassionate environment, giving the caregiver peace of mind. Respite care is designed to meet the physical and medical needs of a resident during a short-term stay.
Hospice Services
We offer compassionate hospice care for those with advanced, life-limiting illnesses. Our team of compassionate professionals will work together to help ensure our patients’ last days are as comfortable as possible and spent with dignity and surrounded by their loved ones.
Behavioral Health
Our secure behavioral health unit was designed to help patients with mental wellness issues including Bipolar Disorder, Schizophrenia, and more. Treatment may include a combination of medication and therapy. Thoughtful, consistent treatment helps patients achieve a better quality of life.
Dementia Care
Our dementia care offers supervised care with meals, activities, and health management for residents. Our activity program focuses on cognitive stimulation, safety recognition, and resident/staff awareness.
Pain Management
We understand that pain can delay and interrupt the progress of rehabilitation. We strive to make our patients as comfortable as possible, through medication and other pain management techniques, to help patients participate in and benefit from therapy. We also provide our patients with educational information and resources they will need to successfully manage temporary, long-term, or chronic pain once they return home.
Dietary Program
- Provides meals to meet specific dietary and therapeutic needs, according to physician orders
- Oral supplemental diets are available
